I shall have no claim, whatsoever, to the volunteers, donors, suppliers and supporters of oxygen4blr.in incase of any mishap and/or unforeseen incidents.
I consent that the use of the oxygen concentrator for the patient is being done at the prescription of a certified medical practitioner and will be done in accordance with the prescribed operating norms shared along with the oxygen concentrator and mentioned on www.oxygen4blr.in .
I, acknowledge and consent that I am engaging with oxygen4blr.in, it's team, volunteers on the following terms and conditions which shall be binding upon me.
